Why magnesium deficiency is easy to over-assume
Magnesium appears in hundreds of wellness claims, so it is easy to assume that every cramp, twitch or tired afternoon means you are deficient. Usually, it does not. Many people eat less magnesium than recommended, but healthy kidneys can conserve the mineral efficiently.
Clinical magnesium deficiency becomes more likely when the gut cannot absorb enough, the kidneys lose too much or a medicine or illness disrupts the balance. Symptoms are broad, which is why testing and clinical context matter more than matching one symptom to one mineral.
What is magnesium and why does the body need it?
Magnesium is an essential mineral involved in hundreds of enzyme reactions. The National Institutes of Health Office of Dietary Supplements explains that magnesium supports normal muscle and nerve function, blood glucose control, blood pressure regulation, protein production, bone health and DNA production.
Most magnesium is stored in bones and soft tissues, with only a small proportion circulating in the blood. This is one reason magnesium status can be more complex than simply looking at one blood result.
What is magnesium deficiency?
Magnesium deficiency, or hypomagnesaemia, occurs when magnesium becomes too low to support normal function. Low intake alone may be partly compensated for by the kidneys. A clinical deficiency more often develops when absorption falls, urinary or gastrointestinal losses rise, or both happen together.
A low serum magnesium level can support the diagnosis, but the result needs to be interpreted with the wider clinical picture.
Symptoms of magnesium deficiency
The NIH Office of Dietary Supplements lists loss of appetite, nausea, vomiting, fatigue and weakness among possible early symptoms. As levels become more severely depleted, symptoms can include:
Numbness or tingling
Muscle cramps, tremors or spasms
Personality or behavioural changes
Seizures
Abnormal heart rhythms
Fatigue, cramps and brain fog have many other causes. They should not be used to diagnose magnesium deficiency without appropriate medical review.
What causes magnesium deficiency?
Several conditions can reduce absorption or increase magnesium loss:
Digestive disorders: Crohn's disease, coeliac disease, chronic diarrhoea and some intestinal surgery can reduce absorption or increase losses.
Poorly controlled diabetes: High urinary glucose can increase magnesium loss through the kidneys.
Long-term heavy alcohol use: This can reduce intake and absorption while increasing urinary losses.
Medicines: Proton pump inhibitors, some diuretics, certain antibiotics and chemotherapy medicines can lower magnesium in some people.
Kidney and hormonal disorders: Some conditions interfere with the kidney's ability to retain magnesium.
Never stop a prescribed medicine because you are worried about magnesium deficiency. A doctor can decide whether testing, monitoring or a medicine change is appropriate.
Who is at greater risk?
Low magnesium is more likely in:
People with chronic digestive disease or persistent diarrhoea
People with poorly controlled type 2 diabetes
People with long-term heavy alcohol use
Older adults
People taking medicines known to lower magnesium
People with conditions that cause ongoing electrolyte loss
Having a risk factor does not mean you are deficient, but it changes how symptoms such as cramps, weakness or fatigue should be interpreted.
How is magnesium deficiency diagnosed?
Serum magnesium is the most commonly used blood test. Because only a small share of total-body magnesium is in the bloodstream, a normal result does not answer every question.
Doctors interpret the result alongside symptoms, kidney function, medicines and other electrolytes such as potassium and calcium. Extra tests depend on the situation and may include an electrocardiogram when an abnormal heart rhythm is suspected or urine testing when the clinician needs to distinguish kidney losses from gastrointestinal losses.
Foods rich in magnesium
Useful food sources include pumpkin seeds, almonds, cashews, beans, lentils, whole grains and leafy green vegetables. Dark chocolate also contains magnesium, but it does not need to become a daily treatment plan.
A varied diet is the best starting point when intake is low. Food can help prevent inadequate intake, but a clinically significant magnesium deficiency may need a specific oral or intravenous treatment depending on severity and cause.
How is magnesium deficiency treated?
Treatment depends on how low the magnesium level is, the symptoms and why it became low. Mild magnesium deficiency may be managed with food and an oral supplement, while severe symptoms, very low levels or an abnormal heart rhythm may require intravenous magnesium and monitoring in hospital.
The underlying cause also needs attention. Replacing magnesium without addressing persistent diarrhoea, medication-related losses or another medical problem may not correct the deficiency for long.
Can magnesium supplements cause side effects?
Yes. Oral magnesium can cause diarrhoea, nausea and abdominal cramping, particularly at higher supplemental amounts. People with significant kidney disease have a greater risk of magnesium accumulating because the kidneys may not remove excess magnesium normally.
Speak to a healthcare professional before using magnesium supplements if you have kidney disease, take regular medicines or have been told that another electrolyte is abnormal. If a new magnesium product is causing loose stools, changing the form, dose or timing may be more useful than assuming the reaction means the supplement is "working".
When should you seek medical help?
Seek urgent medical care for a seizure, fainting, severe muscle spasms or a new irregular heartbeat. Arrange an assessment for persistent weakness, cramps or tingling when you also have a digestive condition, poorly controlled diabetes, heavy alcohol use or take a medicine that can lower magnesium.
Severe deficiency is a medical problem and should not be managed by increasing over-the-counter supplements on your own.
