Male pattern hair loss is the most common cause of hair loss in men. It usually develops slowly, with a receding hairline at the temples, thinning at the crown, or both. Genetics and sensitivity to dihydrotestosterone (DHT) are the main drivers. Stress, illness, rapid weight loss and some nutritional deficiencies can cause a different pattern of diffuse shedding, so the way the hair is changing often gives the first useful clue.
What does male pattern hair loss look like?

One day, you notice that the front looks thinner. Or you catch the crown in a photograph and wonder whether stress, diet or a missing vitamin is to blame. Those things can affect shedding, but they do not usually create the familiar temple-and-crown pattern. The British Association of Dermatologists describes this type of hair loss, also called androgenetic alopecia, as a gradual receding hairline and loss from the top of the head, while the sides and back are usually spared.
Common patterns can give useful clues:
Gradual thinning at the temples and crown: More typical of genetic pattern loss.
Sudden shedding across the whole scalp: Can fit telogen effluvium, especially after illness, major stress or rapid weight loss.
Round or sharply defined bald patches: Can occur with alopecia areata or another localised condition.
Broken hairs with redness, scale, pain or scarring: Need assessment for scalp disease, infection, inflammation or physical damage.
More than one type can happen at the same time. A receding hairline may be genetic while a recent illness or restrictive diet adds extra shedding across the rest of the scalp.
What causes male pattern hair loss?
This type of hair loss is caused by a combination of genetic and hormonal factors. It is polygenic, which means many genes can influence when it starts, how quickly it progresses and how severe it becomes. It is not simply inherited from your mother's side; the British Association of Dermatologists notes that genes can come from either or both parents.
What does DHT do to hair follicles?
The body converts some testosterone into dihydrotestosterone (DHT). In genetically susceptible scalp follicles, DHT gradually shortens the growth phase and causes the follicles to miniaturise, producing finer and shorter hairs over time. Hair loss does not necessarily mean your testosterone is unusually high. The issue is often how sensitive particular follicles are to normal androgen levels.
Is all hair loss in men genetic?
No. A StatPearls review on telogen effluvium describes diffuse shedding after metabolic stress, hormonal changes, medicines, severe illness, surgery, crash dieting, low protein intake or iron deficiency. A 2024 study in Annals of Dermatology specifically looked at telogen effluvium associated with weight loss. Thyroid disorders, autoimmune conditions and scalp disease can cause hair loss too. The speed, pattern and condition of the scalp help distinguish these causes. Sudden, patchy, painful or scarring loss should not automatically be labelled as male pattern baldness.
Can stress, diet or smoking affect hair loss in men?

Stress and illness
Major physical or emotional stress can push more hairs into the resting phase of the hair cycle. The shedding often becomes noticeable a few months later and is usually spread across the scalp. This is called telogen effluvium. It is different from the slow temple-and-crown pattern of androgenetic alopecia, although both can occur together.
Diet and nutrient deficiencies
Hair needs adequate energy, protein and micronutrients to grow normally. Severe restriction, rapid weight loss, low protein intake or a genuine deficiency can add diffuse shedding. If your diet already provides enough protein and micronutrients, adding a protein shake, biotin or extra vitamins will not stop genetic baldness. True biotin deficiency is uncommon, and high-dose biotin can interfere with some laboratory tests. Low iron can contribute to diffuse shedding, but iron deficiency is less common in adult men and should be investigated rather than self-treated. Correcting a confirmed vitamin D deficiency is important for general health, but extra vitamin D has not been shown to reverse androgenetic alopecia when levels are already sufficient.
Smoking
A 2024 meta-analysis by Gupta and colleagues found an association between smoking and a higher likelihood of developing male androgenetic alopecia and with greater progression in some analyses. The studies were observational, so this does not prove that smoking directly causes baldness. Stopping smoking is worthwhile for overall health, but it should not be treated as a way to regrow follicles that have already miniaturised.
How is male pattern hair loss treated?
If you want to slow the loss, getting the diagnosis right matters before choosing a treatment. The British Association of Dermatologists describes topical minoxidil and finasteride as established treatment options for androgenetic alopecia. Both need ongoing use to maintain their benefit.
Minoxidil
Topical minoxidil can slow progression and may improve hair growth in some men. It can take several months before a benefit is noticeable, and scalp dryness, redness, scaling or itching can occur.
Finasteride
Finasteride is a prescription medicine that reduces DHT levels. It can slow hair loss and may help some regrowth, but it is not suitable for everyone and can cause side effects. A doctor should discuss the expected benefits, risks and whether it is appropriate for you. Treatment usually has a better chance of preserving hair while affected follicles are still producing thin, fine hairs. Hair transplantation may be considered in selected cases, but it is a different decision from treating early patterned thinning.
Common mistakes men make when hair starts thinning
Assuming every extra hair means baldness: The pattern and speed of change matter more than counting individual hairs in the shower.
Waiting for a large bald spot: Earlier assessment gives you more time to understand the pattern and discuss options.
Taking biotin for everything: It is not a universal hair-loss treatment and can interfere with blood tests.
Believing oils can change genetics: Oils may help with dryness or breakage, but they do not reverse DHT-driven follicle miniaturisation.
Changing treatments every few weeks: Hair grows slowly, so evidence-based treatments need enough time to judge properly.
When should you see a dermatologist?
See a doctor or dermatologist if you notice rapid or sudden shedding, sharply defined bald patches, scalp redness, pain, scaling or scarring, loss of eyebrow, beard or body hair, or unexplained fatigue, weight change or other new symptoms alongside the hair loss. These patterns may need investigation for a cause other than androgenetic alopecia. If your hairline or crown is gradually thinning and it bothers you, you do not need to wait until the loss is advanced. A dermatologist can usually recognise the typical pattern and discuss whether treatment makes sense for you.
