Can Vitamin D Deficiency Cause Joint Pain? What the Evidence Says

Vitamin D deficiency can cause bone pain and muscle weakness, but that does not mean every aching joint is caused by low vitamin D. Here is how to tell the difference.

Your knees have started aching more than usual. Maybe your shoulders feel stiff too. Then a blood test, social post or conversation brings up vitamin D, and suddenly the pieces seem to fit. That does not mean vitamin D deficiency is a common explanation for every sore knee, stiff shoulder or aching joint. The useful question is not simply whether vitamin D and pain are connected. It is what kind of pain low vitamin D can cause, whether you actually have a deficiency, and whether something else better explains the joint symptoms.

Vitamin D helps the body absorb calcium and maintain the calcium and phosphate levels needed for normal bone mineralisation. It also plays a role in normal muscle function. When vitamin D deficiency becomes significant or persists for long enough, adults can develop osteomalacia, a condition in which bone does not mineralise properly. That can cause bone pain and muscle weakness. The NIH Office of Dietary Supplements covers vitamin D's role in calcium absorption, bone mineralisation and muscle function in more detail. This is where the connection between vitamin D and “joint pain” can become confusing. A person may describe pain around the hips, legs, back or shoulders as joint pain even when some of the discomfort is actually coming from bone or muscle. At the same time, millions of people have knee, hip, shoulder or hand pain for reasons that have nothing to do with vitamin D. So the presence of an ache does not tell you its cause.

Joint Pain Is Not the Same as Bone or Muscle Pain

These symptoms can overlap in real life, but they are not identical. Joint pain is felt in or around a joint such as the knee, hip, ankle, shoulder or fingers. Depending on the cause, it may come with stiffness, swelling, warmth, reduced movement or pain during particular activities. Bone pain can feel deeper and may be more diffuse. Muscle weakness or muscle pain may show up as difficulty climbing stairs, getting up from a low chair or doing activities that used to feel easier. Vitamin D deficiency is more clearly associated with bone health and muscle function than with a specific pattern of ordinary joint pain. The NIH Office of Dietary Supplements notes that inadequate vitamin D can contribute to muscle weakness and pain, while prolonged deficiency can cause osteomalacia. That distinction matters because taking vitamin D for an aching knee caused by osteoarthritis, an old injury or overuse is very different from correcting a genuine vitamin D deficiency.

What Does Vitamin D Deficiency Pain Feel Like?

There is no single symptom pattern that can diagnose vitamin D deficiency. When deficiency is severe enough to affect the bones and muscles, symptoms can include:

  • More generalised bone pain or tenderness

  • Muscle weakness

  • Difficulty with activities such as climbing stairs

  • Aches affecting several areas rather than one clearly injured joint

But these symptoms are not unique to vitamin D deficiency. Muscle weakness, fatigue and body aches can also occur with other nutritional deficiencies, thyroid problems, some medicines, sleep problems, inflammatory conditions and many other health issues. This is why symptoms alone are a poor way to decide whether you need vitamin D.

What Does the Research Say About Vitamin D and Musculoskeletal Pain?

This is where the evidence becomes less straightforward. A 2024 systematic review of 30 studies looking at vitamin D levels and chronic musculoskeletal pain concluded that the relationship remained inconclusive because the studies were highly varied, although some evidence suggested lower vitamin D levels were associated with greater pain intensity. That is an association. It does not prove that low vitamin D caused the pain. People with chronic pain may spend less time outdoors, exercise less or have other health factors that also affect vitamin D status. This makes it difficult to determine which came first. Randomised trials are therefore particularly important when asking whether taking vitamin D actually improves pain. And those results have not consistently shown a benefit.

Will Taking Vitamin D Help Joint Pain?

Not necessarily. If someone has a genuine vitamin D deficiency, correcting that deficiency is important for normal bone and muscle health. But that is different from saying vitamin D is a general treatment for joint pain. In the VITAL trial, researchers followed 1,398 older adults with chronic knee pain. Vitamin D supplementation did not significantly improve knee pain, function or stiffness compared with placebo over the follow-up period. Another randomised trial in people with symptomatic knee osteoarthritis found that vitamin D successfully raised blood vitamin D levels, but there was no significant difference in knee pain improvement between the vitamin D and placebo groups. An earlier systematic review of randomised trials of chronic non-specific musculoskeletal pain similarly found no clear pain-relieving effect from vitamin D supplementation. So the practical distinction is: Correcting a confirmed deficiency makes sense. Taking extra vitamin D simply because a joint hurts may not.

When Might a Vitamin D Blood Test Be Useful?

The blood test used to assess vitamin D status measures 25-hydroxyvitamin D, or 25(OH)D. But this does not mean everyone with an aching joint needs a vitamin D test. The 2024 Endocrine Society guideline recommends against routine vitamin D screening in otherwise healthy adults without an established reason for testing. Testing may still make sense when a healthcare professional suspects deficiency because of the wider clinical picture. That may include factors such as:

  • Limited sun exposure

  • Conditions that reduce fat absorption

  • Previous bariatric surgery

  • Certain kidney or liver conditions

  • Bone disease

  • Significant muscle weakness or bone pain

  • Other clinical reasons that make deficiency more likely

A test result also needs interpretation in context. It should not be treated as a standalone explanation for every symptom.

If Your Vitamin D Is Low, Will the Joint Pain Go Away?

That depends on what is causing the pain. If a person's symptoms are related to the bone or muscle effects of significant vitamin D deficiency, correcting the deficiency may improve those deficiency-related problems over time. But if the joint pain comes from something else, raising vitamin D levels may not make the pain disappear. For example, vitamin D will not repair:

  • An injured ligament

  • Damaged cartilage

  • An overuse injury

  • An inflamed joint from another condition

This is why it is useful to separate two questions:

Do I have low vitamin D?

What is causing my joint pain?

The answer to one does not automatically answer the other.

Looking Beyond Vitamin D

If the discomfort is mainly in one joint, think about what the joint itself has been doing. Did the pain begin after increasing your walking, running or gym routine? Is there an old injury? Does the joint swell? Is the discomfort worse after sitting and improves once you start moving? Does it hurt mainly when you climb stairs, squat or carry weight? Those patterns can point towards issues that need a different approach. Joint health also depends on more than a single nutrient. Appropriate movement, muscle strength, injury history, body weight, recovery and underlying joint conditions can all matter.

What About Calcium, Vitamin K2 and Other Bone Supplements?

Calcium and vitamin D work closely together in normal bone health. Vitamin D helps the body absorb calcium, while calcium forms an important part of bone structure. But bone support and joint-pain treatment are not the same thing. A supplement marketed for bones should not automatically be expected to relieve knee, shoulder or hip pain. The same applies to vitamin K2, magnesium, collagen and other ingredients commonly seen in bone or joint products. Their presence on a label does not tell you what is causing the symptom. If you are considering a supplement, start with the reason you need it rather than trying to match every ache to an ingredient.

More Vitamin D Is Not Always Better

Vitamin D is fat-soluble, which means excessive amounts can accumulate. Too much supplemental vitamin D can lead to high calcium levels and problems including nausea, weakness, excessive thirst, kidney stones and, in severe cases, kidney damage and other complications. The NIH Office of Dietary Supplements covers the risks of excessive vitamin D intake in its professional fact sheet. This is particularly relevant with high-dose products. The 2024 Endocrine Society guideline recommends that generally healthy adults under 75 should not routinely take vitamin D above established dietary reference intakes simply for disease prevention. For adults aged 50 and over who have a medical indication for vitamin D treatment, the guideline also favours daily lower-dose approaches over intermittent high doses. If a clinician has diagnosed a deficiency, follow the treatment plan they recommend rather than copying somebody else's high-dose schedule.

When Joint Pain Needs Medical Attention

Do not assume joint pain is nutritional if it is persistent or clearly changing. Speak with a healthcare professional if the pain:

  • Continues or keeps getting worse

  • Repeatedly limits normal activities

  • Comes with significant swelling, redness or warmth

  • Is associated with prolonged morning stiffness

  • Started after an injury

  • Makes it difficult to put weight on the joint

  • Occurs alongside unexplained weakness or other new symptoms

Seek prompt medical attention for a severely painful, hot or swollen joint, particularly if you also have fever or feel unwell. Those situations need assessment rather than a trial of vitamin D or another supplement.

The Bottom Line

Vitamin D deficiency and joint pain can appear together, but the relationship is not as simple as “low vitamin D causes aching joints”. A significant deficiency can affect bones and muscles and may cause pain and weakness. But ordinary joint pain has many other possible explanations, and research has not shown that vitamin D supplements reliably relieve chronic joint or musculoskeletal pain in people simply because they take more. So if your knees, hips or shoulders have started hurting, do not begin with: Which vitamin D supplement should I buy?
